Case Management and Advocacy

Caring staff at the Post Permanency Family Center can offer your family case management and advocacy services to support you in strengthening your family. Center staff will advocate on behalf of families in adoption, guardianship, and family issues.

The purpose of case management and advocacy services is to support your family in reaching its ultimate level of health and functionality. Case Managers can help you access necessary services, act as a liaison between you and other systems, and provide you with the necessary resources to make the best possible decisions for your family.

Our Case Managers are trained in "best practices" and have excellent listening and communication skills in order to help you access necessary services. Some of these services may include mental health treatment, respite care, summer camps, and other support services.
